I have checked the Toronto police's official website - it gives the forms but no cost-how much is it?

Mugshots and fingerprints can only be taken for indictable offences (this includes hybrid offences at the time of the arrest). If the crown decides to try a hybrid offence as summary- then its mandatory that the police destroy all mugshots/fingerprints taken at time of the arrest, and no, theres no cost to it.
